Overview

Central Placement Specialist is responsible for coordinating all referrals/inquires to the Care One Network in a timely and seamless manner.  Central Placement Specialist must have a working knowledge of the capabilities of all the nursing centers and services in the network.  The Central Placement Specialist ensures an immediate response to all inquires/referrals.  The Central Placement Specialist ensures all the financial pre admission checks are completed and documented in Point Click Care.

Responsibilities

  1. Responds to all referrals and inquiries in a professional manner. This may include but is not limited to selling the network, explaining finances, communicating intake information, and transfer of information to appropriate facility/entity for scheduling of admission/appointments;
  2. Follows up on all referrals and inquiries at specified intervals for the purpose of measuring client satisfaction as well as tracking of client needs;
  3. Completes intake process in a timely manner and coordinates appropriate service delivery;
  4. Maintains a positive working relationship with all referral sources/customers;
  5. Maintains a positive working relationship with all Care One Centers and Service entities;
  6. Effectively markets Care One services via the Central Placement 800 number;
  7. Participates in the on call and weekend rotation;
  8. Performs other related duties as assigned including Benefits Verification, Pre Authorization and Screening for which cross training is required.

Qualifications

  1. Associate or Undergraduate degree in Business or health related field or equivalent work related experience. Background in Skilled Nursing Center admissions preferred.
  2. Experience in call center setting preferred
